چکیده

Background. Several authors have reported cases in which 20`TI scintigraphy demonstrated perfusion abnormality in the septum of patients with left bundle branch block (LBBB) and normal coronary arteriogram. The mechanism of this abnormality, however, remains to be clarified. Methods and Results. To determine whether LBBB itself induces abnormal myocardial perfusion and ischemia and to elucidate its mechanism, we used an in vivo animal model. LBBB was induced by right ventricular pacing in 17 open-chest dogs. We examined myocardial perfusion and glucose uptake using 201T1 and 18F-labeled 2-fluoro-2-deoxy-D-glucose. 201T1 activity in the septum was reduced to 74.7±14.5% of its maximal activity, and mean activity was 86.5 ±5.3% in the free wall (p <0.05). `8F activity in the septum

Gated 99mTc-MIBI myocardial perfusion SPECT in patients with right bundle branch block but without evidence of coronary artery disease

Introduction: We aimed to investigate the effect of right bundle branch block (RBBB) on perfusion and functional parameters in dipyridamole stress/rest Tc99m-MIBI gated myocardial perfusion SPECT (GSPECT) which may be helpful in interpretation of myocardial perfusion imaging. Methods: We studied 73 patients with low pre-test likelihood of coronary artery disease in two groups:  38 patients wit...

Gated myocardial perfusion SPECT in patients with left bundle block but having low probability of coronary artery disease; as compared to the patients with normal electrocardiogram

Introduction: We aimed to investigate of effect of left bundle branch block (LBBB) on perfusion and functional parameters in dipyridamole Tc99m-MIBI gated myocardial perfusion SPECT which may be helpful in interpretation of myocardial perfusion imaging. Methods: We studied 70 patients with low pre-test probability of coronary artery disease in two groups:  35 patients with LBBB and  35 subject...

Cardiac resynchronization therapy homogenizes myocardial glucose metabolism and perfusion in dilated cardiomyopathy and left bundle branch block.

OBJECTIVES We investigated whether cardiac resynchronization therapy (CRT) affects myocardial glucose metabolism and perfusion in dilated cardiomyopathy (DCM) and left bundle branch block (LBBB). BACKGROUND Patients with DCM and LBBB present with asynchronous left ventricular (LV) activation, leading to reduced septal glucose metabolism. Cardiac resynchronization therapy recoordinates LV acti...

Myocardial structural, perfusion, and metabolic correlates of left bundle branch block mechanical derangement in patients with dilated cardiomyopathy: a tagged cardiac magnetic resonance and positron emission tomography study.

BACKGROUND Left bundle branch block (LBBB) influences on regional left ventricular (LV) structure, perfusion, and metabolism have not yet been thoroughly investigated in dilated cardiomyopathy patients. METHODS AND RESULTS Eleven dilated cardiomyopathy patients with LBBB (mean+/-SD age, 62+/-11 years; LV ejection fraction, 35+/-8%) and 7 dilated cardiomyopathy patients without LBBB (mean+/-SD...
